While it is important to achieve a fine result at the completion of braces, it is equally important to hold that result with retainers that are not a huge burden to the patient. Patients often ask how long retainers should be worn. The easy and most correct answer is to wear retainers until you wish the teeth to start moving back toward their original positions. Therefore we suggest retainers be worn forever.
We use only removable retainers since permanent retainers present the same issues as braces, namely, difficulty with cleaning, chance for damage to the teeth and irritation to the tongue. Most patients will eventually want them removed (10 years? 20 years?) and then the issue of relapse is again operating.
The greatest benefit of proper retention is the protection provided against nightgrinding damage (bruxing) or clenching pressures, both of which can affect the teeth, bone and the TMJ. Our office provides a complimentary office visit, once each year, after the retainers have been worn for the first year, in order to check on the fit and effectiveness of the retainers. Retainers are worn nights only after the first year but will only be beneficial if worn properly and periodically checked. Small adjustments, done in a timely manner, can ensure the finest results.
